The Community Care Centers (CCC) aim to provide integrated services on local level to maintain functional capacities of older people, foster healthy ageing and support informal carers. Network activities with various partners in the field of long-term care and health care is another focus of the CCC.
Often the lives of ageing people are marked by increasing need for help and later need for care. They depend on care from relatives. Often there is no option but to move to a care facility. It becomes clear what many peopleare experiencing: New approaches to counselling, support, care and care are needed to ensure the quality of life of older people and also carers.
The new pilot project Generations.Together aims to raise awareness of the issue of loneliness in old age and the associated social, health and economic consequences. The aim of the project is to strengthen the community – in the sense of a “caring community” – by involving important actors at the municipal level and promoting voluntary commitment, thus enabling people in old age to enjoy a better quality of life and a feeling of being part of the community.
